Your “Sweat & Sparkle” online training program will run for 8 weeks.

Billing and methods of payment

Your program will start on a Monday - usually the first Monday available after receipt of your payment and forms but in any event we will need at least 2 working day to set it up for you.

Payment for the initial commitment is to be made via Bank Transfer to the account provided in our email when you submit your application.

Your Commitment

LVK Personal Training recommends that clearance or advice from a GP or medical professional be sought, however you are responsible for seeking such clearance or advice prior to undertaking physical activity, nutritional or supplement interventions with LVK Personal Training to prevent risk of illness or injury.

By purchasing this program, you are making a commitment to your health. You should follow the program and instructions from LVK Personal Training to the best of your ability in order to maximise your results and achieve your goals.

LVK Personal Training will guide and educate you however, you are responsible for your adherence to our guidelines and recommendations.

Termination of Program

The program will expire 8 weeks after it’s start date and once purchased is non-refundable. 

By purchasing this program you acknowledge that all guidelines given to you by LVK Personal Training are guidelines only and that you are legally responsible. You thereby relinquish and waive all responsibility of any health complication that may arise by following the guidelines given to you by LVK Personal Training.

LIABILITY WAIVER

By purchasing this program you confirm that you have volunteered to participate in an online coaching program provided to you by LVK Personal Training which may include, but may not be limited to, resistance training, aerobic exercise, cardiovascular exercise, mobility sequences, or the like. In consideration of LVK Personal Training’s agreement to provide you with a customised training plan, you do here now and forever release and discharge and hereby hold LVK Personal Training harmless from any and all claims, demands, damages, rights of action or causes of action, present or future, arising out of or connected with your participation in this or any exercise program including any injuries resulting there from.

You also confirm that you are at least 18 years of age.

THIS WAIVER AND RELEASE OF LIABILITY INCLUDES, WITHOUT LIMITATION, INJURIES WHICH MAY OCCUR AS A RESULT OF (1) FOLLOWING THE PLAN PROVIDED BY TRAINER; (2) ANY SLIP, FALL, DROPPING OF EQUIPMENT; (3) AND/OR NEGLIGENT INSTRUCTION.

You therefore hereby agree to expressly assume and accept any and all risks of injury, regardless of severity, or death.

You confirm having been advised that an examination by a physician should be obtained by anyone prior to commencing a fitness and/or exercise program or initiating a substantial change in the amount of regular physical activity performed or food consumed.

If you have chosen not to obtain a physician’s consent prior to beginning this program with LVK Personal Training you  hereby agree and confirm that you are doing so solely at your own risk. In any event, you backnowledge and agree that you assume the risks associated with any and all fitness related activities and/or exercises in which you participate.

BY PURCHASING THIS PROGRAM YOU ACKNOWLEDGE THAT YOU HAVE THOROUGHLY READ THIS FORM IN ITS ENTIRETY AND FULLY UNDERSTAND THAT IT IS A RELEASE OF LIABILITY AND THAT YOU ARE WAIVING ANY RIGHT  YOU OR YOUR SUCCESSORS MIGHT HAVE TO BRING A LEGAL ACTION OR ASSERT A CLAIM AGAINST TRAINER FOR NEGLIGENCE OR THAT OF LVK PERSONAL TRAINING’S EMPLOYEES, AGENTS, OR CONTRACTORS.

LVK Personal Training reserves the right to modify these terms and conditions from time to time by updating this notice. You will be notified of any such amendments by directing you to view the same on our website. This is an offer made by LVK Personal Training to you, under which you are invited to browse, peruse and interact with our website for your own personal edification, guidance, information and communication with LVK Personal Training.
